To submit claims under your health insurance contract for items such as healthcare expenses, medical or paramedical expenses, drugs, dental care, vision care, please:
- Download the required form,
- Complete the form in full (health claims form can be completed online) and print,
- Sign it and send the completed form along with your
original receipts to:
