1. Download the application.

2. Enter all required information on the application (be sure to include your overseas mailing address and your e-mail address). 

3. Select single or family coverage.

4. Complete the payment details.

5. Indicate the date of your first departure or check unknown date of departure.

6. Print the form and mail the application via unclassified diplomatic air freight to:

You may send your payment by cheque with your application form.

7. Once your application has been received, you will receive email notification with your policy number. Benefits will become effective on the first day of indicated departure. If unknown, the contract will be in force on date given on your e-mail confirmation.

8. If approved, you have 30 days from the effective date of the policy to examine and return it for a refund of the monies paid, if you are not satisfied.
